SMS consent collection, website redesign, custom-coded forms, advanced CSS or JavaScript development, landing-page creation, welcome automation, legal review, and ongoing conversion optimization are not included. Form visibility and performance may be affected by website code, cookie settings, browser behavior, installed applications, and third-party scripts.

We connect Klaviyo’s onsite form functionality to your website, create up to three branded email signup forms, map subscriber information, configure display and targeting rules, and verify that approved submissions reach the correct Klaviyo list.

Service Scope

Klaviyo Signup Forms & Website Integration is a fixed-scope service for one business, one Klaviyo account, one existing website, one primary email subscriber list, and up to three Klaviyo signup forms.

The service enables Klaviyo’s onsite form functionality, creates and styles the approved forms, connects them to the correct list, configures targeting and display behavior, publishes them on the website, and validates controlled test submissions.

This service covers email subscription collection. SMS consent collection requires a separate Klaviyo SMS setup.

Review the Website and Klaviyo Account

We review the website and Klaviyo account settings directly related to the approved form implementation.

The review may cover:

  • The website platform
  • Existing Klaviyo integration status
  • Klaviyo onsite JavaScript
  • Shopify app embed status when applicable
  • Existing signup forms
  • The intended subscriber list
  • List opt-in settings
  • Available profile properties
  • Existing form display rules
  • Website locations selected for embedded forms
  • Obvious conflicts with other popup or form applications

This is a focused implementation review and not a complete website, tracking, integration, performance, or conversion audit.

Enable Klaviyo Onsite Form Functionality

Klaviyo signup forms require Klaviyo’s onsite JavaScript, commonly called Klaviyo.js, to function on the website.

Depending on the client’s platform, we enable or verify the appropriate installation method.

This may include:

  • Enabling the Klaviyo app embed on Shopify
  • Verifying automatic onsite tracking through a supported integration
  • Enabling the available onsite JavaScript setting
  • Installing the standard Klaviyo.js snippet on a compatible website
  • Confirming that published Klaviyo forms can render on the approved domain

A complete Shopify, WooCommerce, BigCommerce, Wix, Square, Magento, or custom e-commerce integration is not included. The service only addresses the onsite functionality required for the approved signup forms.

Connect the Primary Subscriber List

We connect all approved forms to one primary Klaviyo email subscriber list.

The list is reviewed for:

  • Correct account placement
  • Clear naming
  • Email subscription purpose
  • Single or double opt-in setting
  • Compatibility with the approved forms
  • Existing connected welcome flow, when visible
  • Obvious duplicate form destinations

The client must approve the opt-in method. Double opt-in requires new subscribers to confirm their email addresses before they are added to the list and become eligible for a connected welcome series.

Welcome-series creation or modification is not included.

Create Up to Three Signup Forms

We create up to three branded Klaviyo email signup forms.

Available form formats may include:

  • Popup form
  • Flyout form
  • Embedded form
  • Full-page form

The final combination is selected according to the client’s website structure and list-growth goals.

For example, the package may include one popup form, one footer embed form, and one page-specific embedded form.

Each form is created using Klaviyo’s native form editor and the features available in the client’s account.

Match the Existing Brand

We style the approved forms using client-provided branding and the native controls available inside Klaviyo.

Standard styling may include:

  • Brand colors
  • Available fonts
  • Logo or approved image
  • Form background
  • Borders and corner styling
  • Button appearance
  • Field styling
  • Text alignment
  • Spacing
  • Close icon
  • Success message

The client must provide final text, imagery, offer information, and brand assets before design begins.

Website redesign, original illustration, image editing, copywriting, custom fonts requiring development, advanced CSS, and custom JavaScript are not included.

Configure Form Fields

Each form may contain up to five approved visitor-facing fields.

Fields may include:

  • Email address
  • First name
  • Last name
  • Birthday
  • Location
  • Product interest
  • Customer preference
  • Other compatible profile information

Additional information collected through a Klaviyo form can be stored as custom profile properties and later used for segmentation or personalization.

The client must approve the property names and explain the intended use of any unfamiliar field.

Complex questionnaires, conditional field logic, account registration, file uploads, payment collection, and database lookups are not included.

Add Source and Property Tracking

We configure the standard source information and approved hidden properties needed to identify where subscribers entered the audience.

This may include:

  • Form source
  • Website location
  • Campaign source
  • Offer name
  • Product category
  • Other approved fixed classifications

Hidden fields may overwrite an existing property when the form submits the same property name with a new value. Property names and values must therefore be approved before publication.

Configure Targeting and Display Rules

For popup, flyout, and full-page forms, we configure practical targeting and behavior settings based on the approved audience strategy.

Settings may include:

  • Desktop or mobile visibility
  • Time-delay trigger
  • Scroll-percentage trigger

Where an embedded form is selected, we install its Klaviyo embed code in one approved website location per embedded form.

Possible locations include:

  • Other compatible content section

The website theme or page builder must permit the required embed code.

Major theme reconstruction, development of a new section, third-party page-builder repair, and modifications to protected website code are not included.

Configure the Opt-In Experience

We configure the standard post-submission experience available for the approved list and forms.

This may include:

  • Confirmation message
  • Success step
  • Close or redirect action
  • Client-provided static coupon code
  • Approved next-step instructions
  • Double opt-in confirmation settings
  • Basic subscribe-confirmation branding

Unique coupon generation depends on the client’s store integration and available Klaviyo functionality. Coupon strategy, discount creation, promotional terms, and complex coupon troubleshooting are not included.

Review Consent Presentation

We place the client-provided consent, privacy, and promotional language into the available form areas.

The client remains responsible for providing and approving:

  • Privacy-policy links
  • Marketing consent language
  • Promotional terms
  • Required disclosures
  • Geographic targeting decisions
  • Single or double opt-in selection

After client approval, we publish the completed forms and perform controlled submission testing.

Google Analytics configuration, custom conversion events, third-party analytics, heatmaps, attribution development, and ongoing performance reporting are not included.

The client receives a concise implementation summary identifying:

  • The fields and properties configured
  • The targeting rules applied
  • The selected opt-in method
  • The publication status
  • The controlled test results
  • Known website or platform limitations

The Klaviyo account, website, subscriber data, forms, lists, and associated business assets remain owned and controlled by the client.

Client Requirements

Before work begins, the client must provide:

  • Administrator access to the Klaviyo account
  • Website administrator, theme, or page-builder access
  • One functioning website
  • The approved subscriber list
  • Final form copy
  • Logo, colors, fonts, and approved imagery
  • Privacy-policy URL
  • Final consent and disclosure language
  • Any approved promotional offer
  • Any required coupon code
  • The requested form locations
  • The requested targeting strategy
  • Timely review and approval of the completed forms

Delivery and Revisions

Standard delivery is 5–7 business days after all required access, content, assets, and approvals are received.

One consolidated correction round is included when submitted within seven days of delivery and remains within the original form quantities and approved scope.

A replacement design direction, additional form, different website, new subscriber list, revised offer, or materially different targeting strategy is treated as additional work.
